Request Ban Form

So I clicked on contest ban. The message that is there is the same one for request ban which says to send in a demo to email. Where is the form to contest a ban?

Make sure you click "New Thread" in the upper right hand corner, below all the tabs. That is where you create a new thread to contest your ban
